Pocket of fat/excess skin on the outside of my upper eyelid? (Photo)

The area of protrusion on the side of the upper eyelid could be the tear producing lacrimal gland. It is important to get evaluated by an ophthalmologist before any eyelid surgery.

The bulge that you noticed in the lateral aspect of your upper eyelid is a ptotic lacrimal gland. The lacrimal gland supplies lubrication to your eye, and is usually tucked up underneath the bony orbit of your eye. However, in some patients the gland is positioned below the bony rim, and during an upper blephroplasty it can be sutured back to its normal position to achieve a crisp, smooth upper eyelid. If you are considering upper blepharoplasty, this procedure can be performed at the same time as your upper blepharoplasty. Speak to your plastic surgeon about this. Best wishes.

At the time of blepharoplasty, the incision is often carried lateral into the lateral canthal area to take care of the excess skin.

As we age, it is not uncommon to visualize excessive hanging skin in the upper eyelid area, and excision of this skin may be considered for a more pleasing upper eyelid appearance. In some patients there may be a small deposit of fat in lateral aspect of the upper eyelids. Removal is done using upper blepharoplasty approach. In person consultation with a board certified plastic or occuloplastic surgeon with good reputation in eyelid surgery is recommended. Good luck.

There are two fat pads of the upper eyelids but they are typically not that far laterally(towards the outside)
Consult with oculoplastic surgeons, facial plastic surgeons, or plastic surgeons in your community.
