Hello, offhand it looks like you have a combination of ptosis and dermatochalasis (excess skin on the upper eyelids), both of which are likely causing the lids to feel heavy. There seems to be more skin on one side over the other which may be causing the perceived asymmetry. Even though Upneeq has not helped, it is still possible you have ptosis. Would consider having a consultation with an experienced oculoplastic surgeon to evaluate your eyelids correctly and discuss treatment options with you.

You have upper eyelid ptosis and likely would benefit from ptosis surgery. There is no substitute for a detailed in person assessment. Look for an ASOPRS trained oculofacial surgeon.
