I went for a consultation and disclosed that I am currently taking the medication Prozac and Abilify. They said I need a medical clearance from the prescribing doctor in order to move forward. I do suffer from depression, however the weight I have gained from the medication is makes my condition worst. I had the surgery in the past on my lower abdomen about 11 years ago, however I was not on any medications at the time.

Answer: Smartlipo Prozac and Abilify are not contraindicated for SmartLipo, however, I do agree that you should obtain medical clearance from the prescribing physician prior to your procedure. It is important to ensure that your weight gain is under control prior to having this procedure, otherwise you can gain it back. In my practice we use tumescent liposculpture, sometimes in conjunction with other non-invasive body contouring devices such as CoolSculpting and Thermi to help our patients achieve their unique treatment goals. Once you receive medical clearance for your procedure, I would recommend having your procedure with a board-certified cosmetic dermatologist who is an expert in body contouring.

Lidocaine metabolism competes with Prozac. For a small liposuction or one that uses only a little bit of lidocaine that is not an issue, however, if large areas are being treated and or larger doses of lidocaine are being used accommodations to lower the risk should be done. Usually this means either stopping the prozac or switching to another SSRI antidepressant that does not interact with lidocaine for a week or so around the surgery. Some medications interact with the clearance of Lidocaine from the blood stream, and therefore in such cases there is a higher risk for Lidocaine toxicity. It makes sense to have a depression specialist ensure that your depression is under control before embarking upon any type of cosmetic surgery to ensure that the stress of surgery will not cause your depression to spiral out of control. However, if you are stating that your depression meds are the cause of your weight gain AND you plan on staying on those meds, then perhaps lipo is not such a great idea. First, lipo is not a weight loss procedure. Second, you will continue to gain weight regardless of lipo. The best candidates for lipo are those who are weight stable and nonobese. You should ask your depression doctor to see if other meds can manage your depression without causing weight gain.
