Can your under eyebags come back from gaining weight after transconjunctival blepharoplasty?

I tell all of my patients that weight loss or gain doesn't change the appearance of your eye bags.  The fat surrounding the eye is very unique and does not fluctuate like fat in the rest of the body. If the fat is removed it will not come back. The exception is the outer fat pad that can appear bulgy later in life if more fat comes forward from behind the eye.  This is completely unrelated to weight changes, so no need to worry.  Don't let this change your life as you should enjoy your current result for a long time. Usually not all the fat is removed to prevent hollowness. I suppose that if you gain a lot of weight the fat remaining in the compartments could expand, but I have  not seen that as a problem.

Eyebags may return gradually over many years, but usually this is related to thinning of the septal tissues in the eyelids that hold the fat back in the eye socket. Other factors include eye-rubbing which thins the tissues and allows further prolapse of fat pads, fluid issues/ hormonal problems/ other health problems, sleeping on the face/ sleep apnea... Weight gain may play a small role, and it is a good idea to try to keep it stable. Wildly fluctuating weight causes problems with hypertension, type II diabetes, heart disease, and destruction of joints. If eyebags do return, then touch-up surgery is a consideration.
