I am 29y, 120lbs. A periareolar lift was also done. I don't like how saggy they look today. I also find the size too large for my frame. And I still dont like the areola size. I am currently 32D. I would like a B-cup, with 3-4cm areolae. Can I expect this result from implant removal and lift alone? Or do I need to consider smaller implants? What about no implants and fat transfer later?
Answer: Revision Surgery Without an implant you may not get the upper pole fullness that you once had. The implant selection should be based on a combination of your desired size, soft tissue dynamics, breast base diameter and skin elasticity. I recommend an in-office examination as well as a detailed discussion with a surgeon who you are comfortable with. Finally, make sure your surgeon is a Double-Board Certified Plastic Surgeon certified by the American Board of Plastic Surgery (ABPS), a member of the American Society of Plastic Surgery (ASPS) and the American Society of Aesthetic Plastic Surgery (ASAPS).Best,Dr. DesaiBeverly Hills Institute for Aesthetic Plastic SurgeryHarvard Educated, Beverly Hills & Miami Beach Trained, Double-Board Certified Plastic Surgeon

Answer: Lift You should have sufficient volume to avoid a new implant. An explant with a lift should give you a nice result with a smaller areola and sufficient volume for a B or even C cup size. The lift would have to be a Vertical or even Wise pattern lift to take up the ptosis.

Answer: Breast implant removal and lift Thank you for your question and posting appropriate pictures. He seem to be a good candidate for Breast implant removal and lift. I occasionally support the breast with an internal mesh bra with SERI or Galaflex to provide longevity. A consultation to discuss these options would be necessary.

Answer: Removal of implants and lift Hello! You likely have a good cup size minus the implant that was placed. The lift will be a bit tricky with the implants being subglandular and not submuscular, so make sure you see a board certified plastic surgeon with good breast surgery experience. The lift and implant removal will likely get you to a B cup, and you can always come back later for fat grafting. On the other hand, you could get fat grafting at the time of lift which would give some nice conservative improvement in fullness of the upper breast. I don't think you can go much smaller than 240cc with your frame - that's conservative size.Best of luck!

Answer: Explant The breast lift will certainly help address the sagging look that you don't like. The areola can be downsized during the lift procedure. An examination would need to be done to see how much natural tissue you have to determine if you would need implants. It's certainly an option to have implants added or fat transfer done at a later time.
