How is muscle/tethered scar released after explant? (photos)

I had my implants removed and had a capsulectomy, and a capsulotomy done on the right side (I got CC twice). Now the bottom of my breast crease seems to have contracted upwards and have lost some volume on the bottom outside of my breast. It also seems that my tissue is stuck to my muscle because when I push my breast to have cleavage, the center most part doesn't move. Maybe considering fat grafting but am afraid of cysts.

Thank you for sharing your question and photographs.  Unfortunately without knowing the full details of your procedures it is difficult to offer the best advice, but if you only recently had your implants removed it would be wise to allow at least 6 months to pass to see how your tissues soften and relax.  Your concerns may spontaneously resolve.  If it has been longer than 6 months, revision surgery may be performed through your previous incision to release the tethered scar - cutting it - to prevent its recurrence, or release of the scar in combination with a breast lift to reshape your breast tissue should resolve your noted issues.  Hope this helps.

It could be tethered to the tissues beneath.  I have not had to fat graft this area but I have seen patients with your issue.  That area can be released and have fat graft placed.
