The risk of losing nipple sensitivity is small but is not here. More lifting you need, higher the risk. The risk comes from cutting of tissues (and thus nerves as well) to achieve the lift and reshaping. The more aggressive lift you get, more tissues are cut, and higher is the risk of loss of nipple sensitivity. Ultimately it comes down to how much your breasts bother you. Are they bad enough that you would take on the risk of losing nipple sensitivity? Only you can answer that question.

With breast lifting surgery, risk of loss or change of nipple/ areola sensation is estimated to be between 5 and 10%. Swelling and stretching of the nerves during surgery increases the risk.

I would recommend a donut mastopexy with augmentation- I perform 5-10 of these every week and patients just do not complain of altered sensation or loss of the ability to breastfeed. We know where the nerves travel so I stay clear of those "danger zones."

Yes with breast lifts there is a risk of loss of nipple sensation; this may be temporary or permanent. Most patients sensation stays the same and a few even have increased sensation. There is also possible change sensation with breast augmentation.
Fat grafting will add volume with modest lifting.
Overall the best aesthetic results are will a breast lift and repositioning the breast tissue.

my preferred lift is a lollipop lift when a periareolar won't do (ie greater than 2cm lift required). The lollipop technique was created by a Canadian plastic surgeon named Elizabeth Hall-Findley and treats the breast as a three dimensional structure instead of two dimensions (anchor - lift). Most importantly it does not require the horizontal component scars that the anchor-lift requires and breasts appear much more natural, lifted, and less boxy. Lastly, the lift lasts longer because the lollipop doesn't rely on the skin to hold the breast up, its actually breast tissue thats being brought together to support the breast and hence improves longevity. If you are considering a surgery, I would suggest you to consult a board-certified plastic surgeon. Only after a thorough examination you will get more information and recommendations.

The best indication for you would be a breast lift with implants. The size is decided with the physical exam. The sensitivity of the breast is normally reduced when big breast reductions are performed. Every person is different but normally on the breast lift we preserve the proper amount of nerves so the areola won’t loose the sensitivity.

You really do need the lift to reposition your nipples to their proper place. However, there is a risk of loosing some sensation, but it is low. You might get some improvement with minimally invasive radiofrequency skin tightening like ThermiTight.

Thank you for your question and posting appropriate pictures. He seem to be a very good candidate for a press lift with Implants. Chances of losing sensation in your nipples are less than 5%. It is best of course to have a consultation to discuss options of sizing which affect sensation as well as undergo 3-D imaging.

I do not think plants alone will produce an aesthetically pleasing outcome based on your existing nipple heights. Mastopexy has a low risk of affecting sensitivity whereas significant breast reduction requiring relocation of the nipples is more likely to produce this sequela. Best wishes.
