3 months post op, why aren't my breasts dropping and fluffing? (photos)

Question

Surgery date was 7/22/2014. Was instructed not to wear a band. It's about three months and my left is higher and larger than my right. Although I generally have confidence, this does effect my perception of myself. I had 450 CCs of high profile mentor silicone implants inserted submuscular. Of course I'm going to contact my surgeon and have another post op appointment, but I'd appreciate different opinions.

Your implants should have dropped by now; it is possible that the lower border of the pectoralis muscle was not released fully and this is holding up your implants. It is impossible to know the exact problem without an exam, but you likely need a revision using a  neopectoral pocket technique, release of the muscle and conversion to a dual plane pocket. You can look at before and after photos of a patient with a similar problem under my profile.

I agree with your plan to visit your plastic surgeon.  Occasionally, implants can get stuck in a high position despite the best efforts of patients to massage them down.  Often times, they will eventually fall as the muscle stretches and relaxes.  In your case, it might be that the capsule has formed too far superior and you may need your surgeon to open the capsule on the bottom for the implants to fall into place.  A physical exam will help with the decision making.  Good luck!

Your implants are sitting high by now they should have dropped into the lower poles of the breast. I have my patients massage the implants down and wear a top band to help with the process. You should contact your plastic surgeon regarding your concerns.

I agree with the other doctors that have posted that you may need a revision. Unfortunately, the implants have not dropped properly or settled.The good news is that the revision or re-do operation is pretty straight forward and highly successful. Also, since the muscle has already been lifted, there will be dramatically LESS pain, which is great news.I hope this helps!Dr Chris Saunders

Typically an implant will drop by three months. if it has not dropped at that point, you may need to have it "let down". if they are hard, this may require a capsulectomy to remove the scar tissue, if thye are soft, it may just be the muscle holding htem up and sampling releasing the muscle more can allow them to drop.

Looking at your photos, almost certainly your pectoralis muscle was not adequately released which is limiting the descent. You can see that the bottom of your implant is almost 1 to 1.5 cm above your incision under the breasts on both sides. It would be rare to do a true total  "submuscular" implant placement (ie. no partial pectoralis release or "dual plane" surgery) anymore as it is prone to superior displacement and overly excessive animation of the implant with movement. Unfortunately, I think you will absolutely require a revision to fix this.

Thank you for your question and photo. By three months I would expect your implants to have dropped more. Please see your plastic surgeon and discuss a possible revision. If you contract your pectoralis muscle and feel and see tightening of the muscle below the implant, the muscle may need to be released.
