What bothers me the most is: (1) the marionette lines, (2) jowls, (3) nasolabial folds & (4) downturned mouth. Two plastic surgeons recommended a facelift with fat transfer. But... I am not sure (1) if my face has deteriorated enough for a facelift & (2) if a facelift would help my four areas of concern; plus, (3) I don’t want a fat transfer or other fillers and wonder whether a facelift (without the addition of fillers/fat transfer) would give me enough improvement to make a facelift worth it.
Answer: Facelift You would be a great candidate for a facelift or even mini facelift without undergoing submental platysmaplasty. You have adequate volume so fat grafting is not necessary.

Answer: Volume addition vs lifting If I would have to choose between those two options, I would select one that you do not like. Correction of volume under your lower eyelid and cheeks, nasolabial folds and marionette lines is most likely going to produce better result than lifting by means of face lift. Although you may not like it, fillers may be a cheaper option to see what kind of improvement you may achieve without face lift. Combination would work the best. Good luck.

Correction of volume under your lower eyelid and cheeks, nasolabial folds and marionette lines is most likely going to produce better result than lifting by means of face lift. Answer: Facelift and fat transfer... Your neck is not too bad, so you don't really need a facelift. You can undergo a mini facelift to improve the jowls. However, any lifting technique does not fully correct the lines because you lose fat there as you age. A properly performed fat transfer will complement the mini facelift nicely, look totally natural, and make those lines much softer.

With a deep-plane facelift, where the connective tissues and muscle layers are lifting with the skin, you should have a result that appears more volumized. Some additional volume could be useful, and this generally comes from your own fat (harvested around the belly button). In my practice, I offer this for patients with significant volume loss or very deep folds (nasolabial, marionette). While it is easier, from a recovery standpoint, to do these together, you could do the facelift and then wait an see if you feel that you want more volume. Best of luck moving forward!

Answer: Facelift & What It Addresses Thank you for your question and pictures. First a Facelift will target the skin laxity around the jawline, which create jowls. The marionette lines & nasolabial folds can be soften with a facelift, however that is not is main targeted area. These can be addressed by having some fat transfer or dermal fillers after you have had the facelift to soften them if you wish. A lip lift would be need to rectify the downturned mouth. A valentine lip lift would be beneficial. With the valentine lift, this addresses the corner of the lip. This can be combined with a number of techniques for full mouth rejuvenation. It might be that the depressor muscle is pulling the corner of the mouth down and this may require releasing in part to allow less of a downward turn on the corner of the mouth. Please make sure you find a reputable and qualified plastic surgeon. Good Luck.

A well executed facelift will reposition these tissues back to their natural positions in the face and neck and can also help reposition tissue in the midface, as well. In my opinion, you could have a facelift, and return at a later date if needed to add a fat and/or fillers if needed.My best advice would be to consult with a board certified facial plastic surgeon or plastic surgeon who has experience with facelift and fat injection procedures.
