I had an otoplasty performed on me many years ago, and while I’m happy with the result of my right ear, I’m not about the left one. It is obviously overcorrected and has nearly no anti helix fold at all: there’s just a horizontal line right through my ear. What are my possibilities to try and correct this as this really bothers me? I read that in many cases rib cartilage is used, but I’m really reluctant to use my own rib cartilage.

For an old otoplasty result like this, I would frame the issue as a revision problem rather than a simple pinning procedure. An absent or flattened antihelical fold and telephone-type contour can sometimes be improved by releasing areas that were pulled too tightly and recreating a more natural fold with sutures, cartilage reshaping, and sometimes a small graft. Rib cartilage is one option in selected cases, but it is not automatically required.
Photos help show the contour, but they cannot show cartilage quality, scar tissue, or how much movement is possible. In my opinion, an in-person exam is needed to decide whether local ear cartilage, fascia, or other graft material would be enough and to set realistic symmetry expectations.
