Recurrent gynecomastia from testosterone replacement therapy?

Hello and thanks for your time, I ve had gyno surgery 10 years ago to remove the glands due to pain after taking anabolic steroids. I am not sure if all of it was removed (if its even possible) but i could always feel a small lump above the nipple to the left. First of all does that sound like scar tissue or gland leftover? i ve been on trt for 2 years and after adding hcg that dreaded pain came back along with nipple sensitivity. Will nolvadex fix it?
