Recurrent gynecomastia from testosterone replacement therapy?

Hello and thanks for your time, I ve had gyno surgery 10 years ago to remove the glands due to pain after taking anabolic steroids. I am not sure if all of it was removed (if its even possible) but i could always feel a small lump above the nipple to the left. First of all does that sound like scar tissue or gland leftover? i ve been on trt for 2 years and after adding hcg that dreaded pain came back along with nipple sensitivity. Will nolvadex fix it?

Gynecomastia is a condition characterized by enlarged breasts in males. It usually occurs when the male body produces too much estrogen. Testosterone is the hormone responsible for producing breast tissue. When the amount of testosterone produced by the testes decreases, the amount of estrogen increases.
Testosterone replacement therapy (TRT) is used to treat low levels of testosterone. TRT helps reduce symptoms associated with hypogonadism such as fatigue, depression, decreased libido, erectile dysfunction, muscle wasting, osteoporosis, sleep apnea, and weight gain. Therefore, it is important to discuss these risks with your physician.
If you are experiencing nipple tenderness, it may be caused by scarring from previous surgeries. Scar tissue can sometimes become painful. You should consult with your surgeon about whether additional surgery would help alleviate your discomfort.
You should also speak with your doctor about other options available to you. Nipple sensitivity can be treated with topical creams, oral medications, or surgical procedures.

In gynecomastia surgery surgeons never remove all of the glandular tissue. Doing so leaves indentations with the appearance of disfigurement.
Whatever glandular tissue that is left behind after surgery can still react to hormonal stimulus.
Weather taking an a rum taste inhibitor will improve your situation is uncertain. I suggest you discuss this with your provider who is prescribing the hormone replacement therapy.
If you’re just winging this on your own then I suggest not doing it that way and getting a consultation with an endocrinologist so you can get your numbers and information correct.
