I have a lot of dark spots hyperpigmentation on my back what should i do? I started VI peel but I don't see any dramatic change. I want my back to get clear as soon as possible before my wedding which is in April 2022.is it possible is there any treatment to have clear back without any dark spot??

Fractional laser is an amazing and highly effective treatment in our office. We use the laser treatment to help with acne scars, large pores, fine wrinkles, tissue collagen loss, scars, sebaceous hyperplasia, active acne, and for brightening dark pigmentation. Although there are very strong fractional lasers such as CO2 laser, we use lighter treatments for corrective skincare and maintenance of results. Our Thulium fractional laser, clear and brilliant laser, and Ultra Thulium laser are excellent options in our clinic. We also perform CO2 fractional and erbium depending on skin tone and tolerance for downtime. Patients do have to use a Melarase cream before and after treatment to help further reduce pigmentation after laser. Dark spots can improve with these treatments. I recommend fractional laser for incisional scars as well, especially after plastic surgery.

A combination approach will give the most improvement. These types of scars need a combination of Cosmelan, lasers, microneedling/prp, subcision, salt facials and fillers. It may take a series of treatments and may need some treatments to maintain. I recommend getting a formal evaluation with an acne scarring expert. Best, Dr. Emer

PicoSure General laser is excellent at targeting hyperpigmentation and is one of the only lasers safe to use on ethnic skin types. There is also no risk of hypopigmenting with the Picosure laser which is a risk of many other lasers on skin types 3 and above. Because of this I would recommend a series of 3-6 treatments to fully lift your hyperpigmentation by your goal date. I would also recommend treating with a hydroquinone compound cream or a topical containing TXA such as Skinuva Brite or Skin Ceuticals Discoloration Defense. This will help accelerate the lightening process of the hyperpigmentation. Going forward, I would recommend you to complete multiple in office consultations with board certified experts to discuss your ideal aesthetic goals. This way, they can better assess your face and create an ideal treatment plan for you.

Hi and thank you for your question! VI Peel is indeed among the good treatment choices we recommend for skin pigmentation. There are variants that specifically address skin darkening and pigmentation including VI Peel with Precision Plus booster (as well as VI Peel Body). Other than peels, you can also look into laser treatments such as Erbium laser peel (combined with a brightening cream). Please note that for best results, several sessions might be needed and you need to allow ample time for the results to emerge. I hope it helps and good luck!

A treatment you could try would be a cosmetic laser that addresses pigment issues. A Max G laser treatment would help to diminish the brown pigment that you are wanting to get rid of.

In my opinion, pigmented lesions as your photo shows are likely to respond very well to a powerful IPL or laser treatments. I would recommend an Icon treatment with the MaxG or MaxY handpiece. It would likely give you a 50% improvement after just 1 treatment session, with best results shown after 2-3 weeks. You will continue to get improvement with extra treatment sessions. 
Another option would be a picosecond laser such as the PicoSure.
