In my opinion, if you did not want to go for a surgical route, there is definitely some things you can do will injectable therapy. 
Based on the appearance of your jowls it is possible you would benefit from Kybella first to reduce some of the volume of the jowl fat pads. After that, if the volume is down enough, then radiofrequency treatments to re-tighten the skin may be the next step. Once the skin is tight again then fillers would be an option to smooth out whatever contours are left over. They would also improve the appearance of your oral commissures and reduce the appearance of marionette lines as well.

I’m 47 years old with jowls that are hereditary from both sides of the family. I haven’t had any work done yet. Can someone please give me their honest opinion as to whether fillers would work in my situation or do I need a lower facelift? If fillers would work, which one would be the best to reduce the appearance of jowls?

I would recommend a fractional non-ablative laser such as the Icon 1540 with the XD handpiece. The sooner you treat stretch marks or scars from when they form the better result you will get.

It is difficult to say if this is melasma or not, but, you will likely need a multi-modality treatment. The IPL may have worsened it if the settings used were incorrect. Or it may have also caused increased inflammation leading to more darkening. In my opinion you would likely benefit from hydroquinone based topicals and then adding a fractional picosecond laser.

Recently I have notice some heavy hyperpigmentation on my lower face that really bothers me, I tried an IPL and it got worse. I'm wondering if it is melasma

In my opinion you would get a very good result with 2-3 sessions of a high potency IPL such as the Icon platform with MaxY handpiece. Otherwise a fractional laser would likely improve both the tone (color) but also rejuvenate the texture.

ICU nurse here. I think I have been having an issue with the green hospital scrubs fabric breaking me out (plus sweating in layers of PPE) This is what my chest looks like after using Differin for a couple months daily on entire face, very front neck, sternum and out to the 2 large areas. I want to even the tone/resurface. Hydroquinolone 2% otc does nothing. Differin scar resurfacing gel (without hydroquin) has helped lighten a little. Laser? Topical? What are the options?

You are very young, I would recommend with a few treatments with a radiofrequency device. It will stimulate your body to reproduce it's own elastin. Start with 3-5 treatments and see where you end up. 
If that isn't enough then next step would be to reflate volume with HA fillers.

Depending on what you are looking for, either radiofrequency treatments or fractional lasers may help. I would stay away from IPL for sure. 
If all you need is skin tightening, then radiofrequency has NO risk of harm regardless of skin type. If you want more resurfacing then a picosecond laser such as PicoSure would be the safest for you.

I'm 20 years old. So pretty young but my forehead skin is loose and feels heavy on my eyes. When I touch it. It feels gooby to a certain extent. I am African American though. So most treatments out there aren't friendly for brown skin. Is there any reccomended friendly treatments for me? To tighten my forehead skin.

Unfortunately it is impossible to tell how skin will react with everyone. In general, though, I would recommend trying lighter energy levels first to ensure you understand the treatment and see how your skin reacts to the treatments. It may be that only a couple "lighter" sessions is all you need and you won't have to go through the extended downtime with a "deep" session.

In my opinion, based on the photo you sent, it seems like you may have a bit of an inflammatory reaction here. It is odd that this came from the laser treatment but not unheard of. You may benefit from a trial of steroid creams to see if that alleviates the symptoms. You just need to be careful, however, because the skin below the eyelids is very thin and sensitive so you want to start with a very MILD steroid and for brief period of time (max 2 weeks to start) and re-evaluate a response after that. I recommend trying this only under the supervision of a medical professional.

You can still have treatment with energy devices even if you have had fillers placed. Based on your photo, however, I would actually recommend more of a radiofrequency treatment though laser resurfacing would still be beneficial but with more down time. Radiofrequency devices can still stimulate some collagen, but especially stimulate elastin (provide skin tightening) which may be all you need here.

I had tear trough filler 6months ago, one eye was over filled, I got it partially dissolved 2months ago which left a dent, had the dent refilled a week later, still not looking great and I’ve been left with damaged, wrinkly and stretched out skin under my eye, I was looking at some sort of skin resurfacing/tightening but have been giving a lot of conflicting information about whether it’s right for me, if it’s even possible with pre existing filler or how long I should wait? Help appreciated.

Your best bet would be to ensure your top layer of skin (the epidermis) has completely healed before going to get a spray tan. To be on the safe side I would recommend giving it a full 2 weeks.
