Buttock reconstruction after trauma. I was in an accident in the beginning of April ‘22 left me with a large subcutaneous hematoma on my left buttock. I had it aspirated in the beginning of may, and it turned into a Seroma. The doc that aspirated the hematoma tried to drain the Seroma two weeks later but could not get it all out. I am now trying to find a plastic surgeon to aspirate the Seroma before it becomes encapsulated.

Answer: Can I get fat transfer to correct buttock after seroma (from trauma) is resolved? I do many "fixing" of buttock deformities. You will need a few operations. First ex/incisional drainage of seroma pocket. Than 2-4 months healing with intense massaging. Finally lipo with fat grafting to give both buttock symmetry and a pleasing appearance. Answer: One month old seroma Considering the timeframe you’re already behind the eight ball in terms of getting this to heal without a chronic seroma. In order to avoid a chronic Sarama the tissue surfaces need to be in constant contact and any fluid in the capsule space will prevent this and further cause the formation of the seroma. When I have had seromas in the past I tried to drain them on a daily basis as much as is possible for both mine and my patients schedule. In your case a better option may be to have an interventional radiologist place a drain in the pocket which will allow it to drain continuously minimizing having to go back in daily to have it drained. If you’re interested in having a gluteal fat transfer procedure i.e. BBL then you definitely do not want a chronic Sarama in the area and should make sure that this heals up without a chronic fluid collection. Who was managing you with draining the seroma earlier? Can that person not manage it now? Some plastic surgeons don’t put enough effort into draining these and a very regular basis and the long-term result is a chronic seroma. Once fully encapsulated the seroma can no longer be treated by removing the fluid and can only be treated by open surgery removing the entire capsule and then using a drain while healing. Any major hospital in your area will most likely have an interventional radiology department. You just need a referral to them and medical insurance. Good luck, Mats Hagstrom MD

Answer: Seroma pocket What you have skin redundancy in addition to contour irregularity. What you need is an excision of the overlying pocket skin as well as simultaneous fat grafting to correct any concavity. This ca only be done with an ultrasound guided technique. Based on your pictures, you are an excellent candidate for a Brazilian Butt Lift (BBL). The optimal BBL outcomes are achieved with: 1) use of VASER liposuction to harvest the fat, 2) a closed looped system such as Puregraft to process the fat, and 3) strategic micro-packet transplant technique.
