Am I a good candidate for a lower eyelid blepharoplasty? I’m only 22 years old but I’ve had puffy eyes and dark circles since puberty. I’m wondering if a blepharoplasty would be worth getting as I constantly look tired regardless of how much sleep I get every night.

Answer: Lower Blepharoplasty Common reasons for lower blepharoplasty include tear trough deformity or pseudoherniation of the orbital fat. An in-person exam would provide a more accurate diagnosis, but preliminarily based on your photos it does appear you have pseudoherniation of fat creating "eye bags.” They should also be more prominent when you look up. Transconjunctival lower blepharoplasty with fat repositioning will provide nice results and a better lid-cheek transition in this case. This procedure can be done through an incision that’s hidden on the inside of your eyelid so there’s no scar on the outside that you can see. Consult with 2-3 surgeons in person and determine your comfort level with their approach, demeanor, and before and after results. Good luck! @drdonyoo

You have two bulging spots in your lower eyelids, one of which is bulging fat (which is removed in a lower blepharoplasty) and the other is orbicularis hypertrophy, which could be removed or left based on your goals. The other thing to consider in your case is probably fat grafting to the cheek (not so much that you don't look like yourself), which would help support the lower eyelid and improve your result.
