Good lower eyelid surgery candidate? Age 22 & have had puffy eyes & dark circles since puberty (Photos)

Just because you are young it doesn't mean you can't have lower eyelid surgery. Your photos do show the need for it. Ideally, it would be a combination procedure where some of the fat bag is removed and obtaining some fat and injecting it into the hollow crease under your eye. That way you will have the greatest chance of having smooth contour under the eye. Be sure to see an facial specialist for consultation.

Common reasons for lower blepharoplasty include tear trough deformity or pseudoherniation of the orbital fat. An in-person exam would provide a more accurate diagnosis, but preliminarily based on your photos it does appear you have pseudoherniation of fat creating "eye bags.” They should also be more prominent when you look up. Transconjunctival lower blepharoplasty with fat repositioning will provide nice results and a better lid-cheek transition in this case. This procedure can be done through an incision that’s hidden on the inside of your eyelid so there’s no scar on the outside that you can see. Thank you for your question and photos. Though you are young, the under-eye concerns you describe may be due to genetics rather than aging. You may benefit from a lower blepharoplasty to tighten loose muscles and reposition fat below the eye to add volume and create a firmer, smoother appearance.
By contouring and restructuring the lower lid, the excess puffiness and dark circles you describe can be diminished. I recommend scheduling an in-person consultation with an experienced, board-certified facial plastic surgeon. Yes, based on your photos you look like a good candidate for lower eyelid surgery. You have two bulging spots in your lower eyelids, one of which is bulging fat (which is removed in a lower blepharoplasty) and the other is orbicularis hypertrophy, which could be removed or left based on your goals. The other thing to consider in your case is probably fat grafting to the cheek (not so much that you don't look like yourself), which would help support the lower eyelid and improve your result.

You are a great candidate for lower lid surgery. This surgery can be done on the back of the lower without leaving any scar on the skin. Your natural fat would be reduced and then moved to fill in the tear trough to give a smooth transition from you lid to cheek.

You are, however, too young for a full lower blepharoplasty , but can benefit a lot from under eye fillers or nano-fat grafting.
