hello katie its not very possible to evaluate without photos but there's always a right way to 'correct' or 'improve' things for sure. fillers don't result good in 'pinched' noses, i'll assume its the tip that looks 'pinched' and that happens because the underlying cartilage does not support good the overlying skin and the shape and structure cannot be maintained. adding fillers to nasal creases/folds disadvantages and sometimes even increases collapses the nose and worsens breathing let alone the complication risks. your last question regarding downturning the tip , the answer is yes but with good surgery and material. on the long term -based on what i read - i'd assume you've potential to improve from revision surgery with possible rib grafting or ear cartilage grafting + camouflaging with rib pericondrium + fascia. these can be done in a single setting. in good hands hopefully cosmetic appearance + functionality of your nose can potentially improve for the good (not temporarily like the last time but permanently) hope my answer give insight , good luckdoctor be+ clinic
