2 years post op. Is my implant bottoming out? Scar looks worse and implant can be felt. (photos)

I think it is possible for the implant to bottom out given your frame and the size of the implant. Especially with the weight fluctuations you may have always had this issue it just became more bothersome. Combining fat grafting, smaller implant size can help avoid these issues in regards to a revision. It all depends on your goals and what is bothering you. Hello! It seems that you have implant malposition. This can be due to many reasons. You should definitely see a plastic surgeon that specializes in breast revision surgery. This is a complex problem and you need a true expert to fix this issue. Your solution involves placing likely smaller implants and a support system such as Galaflex.

It does seem like it is bottoming out, but mostly on the medial bottom side of the breast. And it also seems like the implant is larger on that left side. You obviously need a pocket revision and I believe you would benefit from a peri-areolar lift as welll. I would even consider reviewing implant size on that breast…..
Conclusion: A pocket revision with a mesh, a smaller implant, plus a small peri-areolar lift would address all your issues.

It looks like your implant is bottoming out on the left. The change in shape may also be related to capsular contracture. Repair of the pocket, possibly with internal mesh, and implant exchange is likely the fix but you should be seen in person to evaluate.

bottoming out occurs when the implant falls below the current breast crease or inframammary line. It's more likely to happen with inframammary incisions, which is why I try not to use the inframammary approach to any breast surgery.
Bottoming out can also occur if an implant is too large for the breast pocket. That is why it is important to choose an implant size that's correct for your body. I perform numerous measurements in the office to determine optimal implant size for my patient's bodies.
If you are concerned, please check it with your plastic surgeon for reassurance and to further discuss your options and possibility of revision.

Your implants are a little large for your tissue and yes, you are bottoming out from the weight. I would suggest wearing an underwire bra for support. If you wind up having surgery, you might want to go down in size.

The left side went to left side too much. It seems to change the implant to smaller one and better to check the pocket. Also, if you have any symptoms like pain or discomfort then better to check up the breast.

If you are thin your implants may be stretching the skin, and without support the shape is affected. You may want to consider adding mesh to give further support to the tissues to prevent this from happening again.

You have several things going on. Your implants are large and have dropped and stretched your skin. This is one of the potential complications when placing larger implants.
Revision with (suturing) of the medial and lower internal position of the breast and the placement of ADM (acellular dermal matrix-an internal patch to bolster the tissue to stop it from dropping again and also to pad the medial portion so you do not feel the implant as much). You may also benefit from a periareolar (around the nipple/areola) mastopexy (lift).
