The subpectoral placement of your implants did not cause this problem, as evidenced by the normal position of the left implant. Therefore, placing them over the muscle would not be a cure, especially since the lower half of the implants, where the problem is happening, are actually subglandular already. In fact, it would be introducing a new problem; operating on the other breast that has no problems. Also, subglandular implants have higher rates of implant palpability, visibility, and capsular contracture. Instead, you need an experienced surgeon who can resize the right pocket using not only stitches, but also mesh to fix the dropout once and for all.

When implants are placed, tissue is cut to generate the space to allow the implants to exist. 350 cc implants are large. The only thing holding up your implants is the skin envelope and as you can see the skin envelope does not have the mechanical strength and the implants have dropped down. You need to consider smaller implants behind the muscle and will most likely need a circumareola approach to correct the asymmetry of the nipple-areola complexes.

Thank you for your question. A large portion of my practice is dedicated to breast revision surgery, and I have been in practice over 28 years. Please do not have your implants placed on top of the muscle. I have a whole gallery of long-term changes associated with implants on top of the muscle leading to slide down, wrinkling and knuckling, and capsular contracture. You should be able to have an excellent result with a properly performed right sided inferior internal bra-capsulorrhaphy. I wrote the very first paper on this in 1999. Additionally, you will need circumareolar repositioning, ie a donut type lift to lower the right areola and elevate the left areola. Best wishes to you.

Actually, you have more support keeping the implants behind the muscle, better upper breast fullness, and hide the implant edge when the implants are behind the muscle. You might need to go down in size some and use mesh for better internal support. Do not go larger.
