After revision necklift failed to fix contour deformity, I was referred to a top PS's who routinely performs resection of bulging digastric muscle which I confirmed is my problem. My current PS thinks corset platysmaplasty might also be needed. I'll see expert at 6-9 mos postop for eval, but hoped for input from RS dr's who've done it, what this can cost and how difficult recovery is. It's clear the deformity requires surgery and I'm hoping a conservative approach will yield good results.
Answer: Not a simple fix Based on your previous posts and if I recall your case correctly, your neck seems skeletonized and everything which is done will be visible through the skin. In order to avoid this, the outer fascial layer over the platysma preferentially is kept intact and all the work being done is subplatysmal. “Shaving” the digastic by going through the platysma from the outside will likely result in contour irregularities because you have little subdermal fat to camouflage it. A platysmaplasty will almost always be required as well as wide skin undermining of the neck through a post auricilar approach in addition to the submental incision, which will likely result in a better result. A limited conservative approach will likely have limited results and I would be ready for wide exposure of the problem. Because of these factors, expense will be high and I would ask to examine you and see all previous op notes and post op notes and all pre/post op photos before even proceeding.

It is not clear if you already had your digastric muscles trimed. It is also not clear if you already had corset platysmorrhaphy during your first procedure. If you want to get a valid answer to your question, you will need to be seen. You would also need to bring with you the operative notes from previous procedures so that the plastic surgeon who sees you can have a clear idea of what was done to you. Ideally you would bring pictures of you prior to the first neck procedure. After this visit the plastic surgeon can give you a clear idea of what needs to be done and the cost. In general any time I do any type of sub platysmal work, I do a corset platymorrhaphy. This would be necessary after removing subplatysmal fat or trimming the digastic muscle bulge.
