During March od 2021 I fell down the stairs and I believe i experienced fat necrosis. I was told that a BBL would not be helpful for me because of the way the dent occured. I spoke to another plastic surgeon who mentioned Sculptra but he was also uncertain if that would help. Are those the only two options to fix this? It's a small dent but it truly does bother me.

Well, if you are also considering or willing to have a bit of a augmentation in your buttocks, with more fullness and perkiness/projection, then the most reliable and permanent option would be buttock implants. Please keep in mind that an extremely few number of board certified plastic surgeon specialize in buttock implants, so you may have to travel far and away from home to find this particular type of surgeon. BBL, targeted fat graft, Sculptra or Renuva is what I would recommend without taking a look in person.  I think BBL would be ok but perhaps a bit much unless bigger buttocks is what you would prefer.  If not, then just fat graft targeting the dent.  Wire subcision can also help release the area and fat graft can keep it from collapsing in again and forming a dent.   Sculptra is ok but Renuva is also a shelf item that may work better. Considering that fat necrosis/scar is the cause of your condition, I believe that fat grafting would be the most appropriate way to correct it. However, considering that is relatively small area, injection of filler like Sculptra, may be viable temporary treatment. Good luck.

based on your photos, you look like a good candidate for a BBL. Liposuction of your abdomen, back and flanks should give you more curves and hourglass shape and fat transfer to buttocks should give you more projection. I would suggest you to consult a board-certified plastic surgeon because only after a thorough examination you will get more information and recommendations.

I agree that your trauma most probably caused some fat necrosis leading to a dent.  I do not feel that Sculptra will change this area as well as a fat transfer.

It would be helpful to see pictures from different views.

To me this looks like cellulite and whether it is just baseline cellulite or fat necrosis with scar tissue information in some ways doesn’t change the surgical approach.

It’s probably not a loss of volume but rather contractile forces pulling in on the skin.

Subsicion with the addition of volume whether it be fillers or small amounts of grafted fat it’s probably your best shot though no guarantee that it will respond.

To me this looks like baseline cellulite and perhaps you noticed it after the injury.

Seeing other views could help confirm this.

Simply adding volume is most likely not going to fix the problem.
