What are my options for less of a square butt? (Photo)

You are in good company with many others who had high hopes for BBL but instead received disappointing results (lumpiness, unevenness, lack of projection in the central and/or lower buttock, post-deflation sagging, etc.) as most of the fat melts away. It is not quite as much the quality of the doctor nor liposuction technique but rather the quality of the procedure that is a large part of the problem. Thus, going back for round 2 BBL most often produces the same disappointing results but at another financial loss. Additionally, you clearly have no good fat available for liposuction and fat transfer.

Unfortunately you “don’t want implants” because buttock implants indeed provide a more reliable, predictable, relatively safe, and permanent result, especially for adding projection to the buttock and often smoothing out lumpiness. Perhaps you have been misinformed about buttock implants and may want to reconsider once you learn more facts about the advantages. Please keep in mind that an extremely few number of board-certified plastic surgeons specialize in buttock implants so be sure to consult with one that is reputable and experienced. Unfortunately you may have to travel long and far away from home to find this particular surgeon. Glad to help.

Thanks for your question and attached photo! It is difficult to say with complete certainty since I have not examined you myself, but I would recommend fat transfer to the hips as the best option. You may consider being evaluated for any areas on your body with excess fat, possible waistline or inner thighs again. There are filler options as well such as Sculptra and Renuva that can mimic fat, however natural fat transfer will give you the most volume replacement. Make sure to consult with a board certified plastic surgeon, good luck!

Ok consider two products that are off the shelf for issues like yours: Sculptra an/or Renuva. Both can help enhance shape of buttocks when patients do not have enough of their own fat. Best Wishes!!

Hello and thank you for your questions and your picture. One view does not provide enough information to adequately assess the buttock and hip areas. Since you do not want buttock implants the other option available is to undergo some fat transfer to the areas in order to improve your shape and contour. However, as you point out, that is highly dependent on whether or not you have enough fat to make a difference. I would start with a consultation with a plastic surgeon to get an adequate physical exam and come up with a treatment plan that is tailored to you. I wish you the best.

Converting a square butt to a round but is the most common reason to undergo a BBL.
Based on your pictures, you are an excellent candidate for a Brazilian Butt Lift (BBL). The optimal BBL outcomes are achieved with: 1) use of VASER liposuction to harvest the fat, 2) a closed looped system such as Puregraft to process the fat, and 3) strategic micro-packet transplant technique. You can improve the contour of your bottom significantly by having proper liposuction that lowers the transition point further than done by your previous provider. (The transition point is a line which differentiates where people need fat removed and where it should be added when doing liposuction with or without a BBL.)

To me the goal is to create a long continues swooping S-curve from the chest down to the thigh.

If you look at your hip dip there’s a substantial amount of fat or bulging just above it. If that is reduced then you’ll come much closer to having an ideal S- curve. That is the easiest way to get an improved figure but adding fat to your hip dip and buttocks will help as well.

Fat can be harvested from previously treated areas though you’ll get less fat out and any untreated area can be used to harvest fat this can include under arms, back, arms, inner thigh etc. etc.
