High or low profile implant? Some sagging and very little breast tissue. (Photos)

Thank you for sharing your question and photographs.  In general for primary breast augmentations most patients benefit from either a moderate/moderate plus/high profile implant.  These offer an improved breast volume and shape but with the projection needed for a noticeable change.  The best size and style will depend on an in-person examination of your key breast parameters but based on your goal pics I would likely recommend a full profile.

Cup sizes are not standard across bra makers, and implant sizing is NOT based on height, weight, BMI, or desired cup size.  This is something you will need to do IN PERSON with an experienced surgeon to determine what works well for you, and to get advice about if what you like is possible and what to expect with regards to results.  Be sure to discuss your goals and concerns with your board certified plastic surgeon and consider the pros and the cons. Thank you for your question! I definitely wouldn't recommend a low profile implant to achieve your goals. You appear to have a relatively narrow base width so I would recommend a moderate or high profile implant to achieve your goals. The only way to really determine how many cc's you need to get the look you're hoping for is by doing an in-person consultation and trying on implants.  That will definitely allow you to see a more realistic image of what you can achieve.  Good luck!

From your pictures, you look to be a good candidate for breast augmentation. Based on your pictures and desired results, I would recommend that you look at a medium profile implant. A high profile might give you too much upper pole roundness and a low profile implant of sufficient volume will probably be too wide for your chest dimensions. Obviously, you would need an in person consultation to establish your measurements to determine the best choice. In addition, to profile, You should also look at the different options for implant gel thickness. You may want to consider a more cohesive or thicker gel to decrease the chance of implant palpability and wrinkling since you are so thin.

Questions regarding which size, shape, and type of implants to use are best answered in consultation with a board-certified plastic surgeon who can examine you in person, listen to what you area trying to accomplish, and then review your options with you.  While you describe yourself as "sagging," it does not appear from your picture that you are sagging enough to warrant a breast lift procedure.  I believe that you can get a nice result from implants alone.  I think a moderate plus implant placed under the muscle would give you a very nice result, again, based on your picture.  As to what volume to use, that is where I would need to see you in person.  Pictures are nice and helpful, but never as good as an in-person consultation.

for a breast augmentation.  The size, profile, and shape of the
implant is based on your desired breast size/shape, your chest wall
measurements, and soft tissue quality.  This decision should
be based on a detailed discussion with equal input from both you
and your surgeon.  This entire surgery
can be performed with a small incision technique.  Make sure you
specifically look at before and after pictures of real patients who have
had this surgery performed by your surgeon and evaluate their results.   The most important aspect is to find a
surgeon you are comfortable with. I recommend that you seek consultation with a
qualified board-certified plastic surgeon who can evaluate you in person.

Dear r007m,breast implant type and sizing selection is a complicated process and requires a detailed consultation that includes a discussion of your goals, a detailed examination, and a lengthy discussion. But the decision will always start and end with the patient. When it comes to sizing, we have all of our patients find 3 photos of their desired breast size. We ask for naked breast photos because clothing can obscure the true size (push up bras). Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements.Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match.Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process. There's an easy solution to your question!At the my consultation, I like ladies to try on sizers to see which "look" they like best.  We use both moderate plus and high profile.  Often the deciding factor is which balances your hips best and complements your overall figure best.  And, an implant you should seriously consider is the Ideal Implant which gives you the best of both worlds.  It is a structured saline implant that feels like gel but has a higher safety profile and the opportunity to vary the amount of saline which would be a good choice for you.  It does not require an MRI for monitoring leaks.  And, they are beautiful!Good luck!In good health,Ellen Mahony, MD

Based on the photos, you could choose a profile either moderate profile (in Natrelle) to moderate profile plus (in Mentor). 
The size of the implants is based on a combination of a patient's goals and objectives and her anatomy. In order to provide a natural looking augmentation, the surgeon needs to evaluate many factors including soft tissue coverage, skin laxity, chest wall width, breast imprint width, the shape of the breasts and relative level of constriction to ensure that the breast implants are appropriate for your body. 
Consult in person with an experienced and expert board certified plastic surgeon to get an understanding of your options.
