I'm planning to have revision rhinoplasty done a year after my primary rhinoplasty. When I go for my revision consultation, I'll have to show the new surgeon my primary rhinoplasty operative report. How do I go about asking my original surgeon office for the operative report without making him feel offended or disrespected? Thanks.

Answer: Need Operative Report
A copy of your medical record is your right as a patient. You can have your revision surgeon's office request a copy or you can get a copy yourself. Either way you will have to sign a medical release form. The paperwork is usually handled by the office staff and your original surgeon may never know you requested it.
However, you should discuss it with your original surgeon so he/she can understand what went wrong between you and why you went to another surgeon.
