You are a good candidate for Gynecomastia. Removing the gland and doing some liposuction would get you a great result. Breast lifts are only needed in severe cases when a lot of breast tissue is present and sagging occurs past to a certain length or measurement.
Hi- thanks for your question. Based on your photos I wouldn't recommend an initial skin excision gynecomastia procedure. The skin often retracts, and based on your skin excess I don't think you would necessarily have "droopy" tissue after. Tight is a relative term- of course lifting and adding muscle and decreasing body fat will help with overall appearance and dealing with the skin. It appears in the picture that you have a small amount of redundant skin and pretty good skin tone. The elasticity of the skin is the force that allows it to snap back once the fatty tissue underneath is removed by liposuction and the breast tissue removed by a pull through minimum incision technique

The skin laxity will limit the improvement with traditional open glandular excision and fat removal using liposuction. Loose skin vo cause a bulging contour that will simply make the outcome look like you were partially treated.With your pectoralis muscle and skin under tension by putting your arm behind your head after the treatment it will be flat if it’s done correctly but most likely your chest to a bulge when you put your arm back down.Male chest lifts are a bit tricky and some surgeons do them better than others.The same principles used in a female breast lift surgery cannot be applied to gynecomastia if glandular excision is going to be done at the same time.Consult with a few plastic surgeons in your community to see what each one has to say.In the end it will be a decision between having scars with an improved contour or a partial result with less scars.Best,Mats Hagstrom MD

You do not appear to have a significant excess of skin that we see in massive weight loss patients. Those patients tend to require full skin reduction or breast reduction with nipple graft vs. the less invasive gynecomastia surgery.Consult with a board certified plastic surgeon in your area to discuss your options. The only proper treatment for gynaecomastia is surgical removal of the gland. Stopping to smoke weed / tobacco is important for your health as well as important for Surgery because this habit complicates anaesthesia during surgery , also causes more bleeding.
