In this case, there may be an asymmetry between the bilateral muscles, both in motion (ie while laughing) and at rest (ie, without moving at all). This is normal. In our practice, we are most likely to utilize a dermal filler or threads for nasolabial folds. Dermal filler can help give an appearance of volume in areas where volume has been lost, and threads can help give a lifted and toned apprearance. Botox can be used (as an off label indication) to relax muscles around the mouth, which can change the motion of the mouth and/ or lips to some degree. This would most likely be an expected outcome of the Botox based on your desired outcome. In our practice we do not place Botox directly into the nasolabial fold area. For our patients who are looking to change the shape of their mouth by bringing the corners of the mouth up, we inject Botox into a muscle called the DAOs in the lower face. If patients desire improvement in the appearance of a gummy smile, there is an injection site near the side of each nostril that helps the lip not pull up as much with smiling. Of course, every patient's individual anatomy is unique and it is best to be seen by a trained Registered Nurse in a Board-Certified Plastic Surgeon's office prior to treatment. Good luck!
