Rhinoplasty revision and scar. What can I do to improve this? (Photo)

1st rhinoplasty to reduce nose size (alarplasty & silicone). Silicone slanted, revision rhinoplasty (replaced silicone & ear cartilage for tiplasty). I want to remove silicone bridge and ear cartilage from my tip, & get revision alarplasty to improve scars and reduce size. Due to thick sebaceous skin, surgeons don’t recommend it and tell me that even with alarplasty, the scars will probably will be the same and the size of my nose will not reduce. What can I do???
