Based on your photo, you are a great candidate for rhinoplasty revision. It's unclear what was done the first time because the bump is so big. But you should be able to have a nice improvement in experienced hands. Be sure to see a rhinoplasty specialist that can also do computer imaging at the same time.

You would definitely benefit from a rhinoplasty/septoplasty. You need the tip rotated up and the dorsal hump must be reduced.

That bump on the bridge can be taken down, and the tip elevated so the nose isn't so long from top to bottom. How *much* to elevate the tip, or lower the bump, or make other changes? We figure that out with morphs, well before surgery. I'd love for you to see some excellent professionally-designed morphs of what could possibly be done with your nose. Morphs could also help you identify better just what's bothering you, and help you set a goal for the rhinoplasty that's accurate for your tastes. Profile and three-quarter views would be particularly important in morphing your nose. (Side note: in my opinion, morphs should really be done by the surgeon, or he should direct an assistant as she makes the morphs. Morphs should be made with a constant eye to what actually *can* be done in surgery, for that particular nose, and the surgeon has that information and judgment best.)

Finally, remember that rhinoplasty is an exquisitely difficult operation to get right, and you should only have surgery if you are able to make yourself very confident in your surgeon's skills. The changes that your nose needs require advanced techniques, and skill that most plastic surgeons don't possess with expertise. If deviation and bump of the nose newly developed or aggaravated after septoplasty, there is a high possibility that the septum was not properly corrected or saddle nose developed. I recommend revision septorhinoplasty to solve these problems.

Revision rhinoplasty is one of the most difficult procedures to perform correctly in the entire field of cosmetic surgery. Besides, a deviated septum, there are several other issues on the inside of the nose it can cause nasal obstruction, so an in- person consultation is probably going to be required. Choose your second surgeon wisely based on extensive experience producing natural results. Digital computer imaging would also be helpful to understand what can be accomplished with your nose upon your facial features.
