I have severe bilateral macromastia with at least 900-1000 g of excess breast tissue per breast. There are no palpable breast masses. The nipple to sternal notch distance is 37 cm on the right and 38.5 cm on the left. The nipple-to-inframammary fold distance is 16 cm bilaterally. There is no palpable axial lymphadenopathy. I have lost a lot of weight. I am currently 235 lbs at 5'8. I want smaller plumper breasts. I am having a breast reduction/lift. I am 40 years old with one child.
Answer: Breast reduction You are a great candidate for a breast reduction. You will see a dramatic difference with reshaping, perkiness and proportion. No need for an implant. Also, it is usually helpful to add liposuction of the front bra roll (side breast) to maximise the cosmetics of the breast.I would not hesitate to proceed. Thank you.

While a breast reduction will lift the breast and reposition the lower pole, it does little to restore upper pole fullness. This can generally only be done with a breast implant. In order to do this safely, you will need to have the reduction mammaplasty first, and then place implants at a later time.

Answer: Surgery For you, I suggest doing the reduction only and avoid implant. If implants are added, they need to go behind the muscle and be small. Your now sagging skin has been stretched and will not support heavy breasts again. Going down to the smallest safest size is best for long term good results. Implants can be added later if needed.

Answer: Implants Thanks for your question! I would not suggest implants with an initial surgery. I think you would benefit from a dramatic breast reduction. Then, you should wait and see how you feel with your new breasts. You may be happy with what remains. Good luck!
