Thank you for your question. As a guideline, patients who have over 20 cm from the inframmary fold to the nipple are a candidate for the free nipple graft. If you wonder whether or not a nipple graft is necessary it probably is a good idea to get a consultation with an experienced surgeon
It is difficult to tell from the pictures. Generally speaking, a free nipple graft is only necessary if the distance from the underside of the breast (the inframammary fold) to the nipple is 20cm or greater. Your surgeon should measure this preoperatively and let you know what your options are. From the pictures, it does not appear that you would need a free nipple graft, however only an in person physical exam and measurements would be able to tell.

Hi there,The short answer would be no. It is very uncommon nowadays to perform free nipple grafts due to the almost permanent lack of sensitivity to the nipples and very frequent discoloration that occurs with the graft that would need pigment matching tattoo in the future.I would recommend seeing a few plastic surgeons in your area and get multiple opinions.All the best

It depends on the provider. I have done them, although less frequently. Either option is valid for getting the reduction done. Personally, I would pursue a pedicle (non-nipple graft option) as I am comfortable in this scenario. Either way, you will be very happy with the results. Thank you.
You should be able to have a breast reduction without a free nipple graft. It's important that your surgeon understands what you are hoping to accomplish from surgery so that appropriate benefits and risks can be discussed. Visit with a board certified plastic surgeon near you to learn more.
In you, I would avoid a free nipple graft and leave you a little bigger to be safe. If you want to go down to an "A or "B" cup, then you will need a free nipple graft or at least 2 surgeries at least 1 year apart.
