Question

I had an open septorhinoplasty performed in 2022. Upon speaking to my doctor regarding the internal structure as well as the deviation & dorsal bump, it was suggested that I also have my nose shortened to be more aesthetically proportionate. I did have my nose broken with all above focused on. Present day, I have deviation/dorsal hump. My Dr. did explain that cartilage has memory. I understand w/surgery there are no guarantees of sucess/perfection. Any advisement of how to proceed is welcome

Based on your photos, you appear to have very thin nasal skin. When people have thin nasal skin, there can be exaggerated shrink-wrap of the skin onto the underlying cartilages and bone of the nose. Since you have thin skin, I believe the expertise of your revision rhinoplasty specialist would be paramount. If you are looking for a smaller and less distracting nasal appearance, before you consult with another surgeon, please consider getting a copy of your operative report and your preoperative photos for review. Revision rhinoplasty is more difficult than a primary rhinoplasty, and one of the most difficult procedures to perform correctly in the entire field of cosmetic surgery. The residual dorsal hump that you have can be filed down. You also will require osteotomies to straighten and narrow your nasal bones. You also. Have a concave upper lateral cartilage on your left side in the mid portion of your nose, which will require cartilage spreader graft for improvement. It's very important to know how much cartilage is left over on the inside of the nose for grafting purposes. Best to obtain a copy of the operative report from the prior surgery. If there's not enough cartilage in the nose, you may require a graft from the ear.
