Scar on left eye is barely visible but right eyelid forms weird wrinkles and scar is still very noticeable. I am tired of looking like this, I got PRP procedure to improve healing but it doesn’t seem to help. Do I need to wait longer or I should get a revision?
Answer: PRP is a scam in my opinion. It has no role in helping an eyelid like your. You need revisional surgery. Unfortunately you have PUBS-post upper blepharoplasty syndrome with upper eyelid ptosis, upper eyelash ptosis, compensatory eyebrow elevation, and asymmetric high upper eyelid creases. The heavy upper eyelid shadows the corneal surface making the eyes look dull and lifeless. Upper eyelid massage will actually make the upper eyelid ptosis worse. Kelo-cote is a silicone based ointment touted to help improve hypertrophic scars. Only problem is that well design studies to not support the value of these types of products. You need revisional surgery for these eyes to make them look bright and vibrant. The surgery you need is far beyond the skills of the surgeon who performed your surgery. The creases need to be lowered, and made symmetric. The upper eyelids ptosis needs to repair the white line disinsertion of the levator aponeurosis that was caused by your blepharoplasty. An anchor blepharoplasty is also needed to address the upper eyelid lash ptosis. When the upper eyelid ptosis is corrected, your eyebrow will also relax helping you to have a much more youthful look to the upper eyelids. This type of repair is highly specialized. Please study the video and the citation below. You are ready now for this revision.

Answer: Scarring blepharoplasty Indeed, there is an asymmetry. This is likely to be as a result of the way the two eyes were treated, meaning there may have been more tissue removed from one eye than the other, causing tension on the eyelid. In my opinion, PRP would not correct this problem. You would need a proper consultation with your surgeon to discuss your options for scar revision surgery. It's likely that if you go ahead with this, you might also require surgery on the other eye as well, despite the fact that the scarring has healed well.

Answer: Blepharoplasty wounds Hello, the right upper lid wound appears to be lightly lumpier causing some wrinkling of skin. You may benefit from a combination of kenalog injection to the scar to help soften it but may also need some skin revision at the scar line.

Answer: Scarring After Upper Eyelid Surgery Thank you for your question. As people are never perfectly symmetrical, one eyelid maybe heavier on one side to another, therefore the scars never being exactly the same. The scars look like they are pulling however creating the appearance of wrinkles. You can try to massage and soften the scars, or use a product could Kelo-cote which is a silicon gel which helps scarring. Please return to your surgeon for regular follow ups. All The Best

Answer: Blepharoplasty Revision Thanks for the question and posting photos. At 7 months, most of the swelling should have gone away from a blepharoplasty and the results should be pretty close to final. It typically takes about 1 year for the scar itself to become its final appearance. Based on the photos, it appears the right side wrinkles/pleats may be due to some residual skin excess. A revision may be warranted to remove this remaining skin excess. As for the scars themselves, many surgeons will wait for 12 months before doing any surgical revision though treatments such as lasers or scar gel/cream can be applied before then. You may consider discussing this with your surgeon to see if a revision is needed and to discuss other modalities for treating the scar. Best wishes, Rohit Jaiswal, MDPlastic Surgeon; Las Vegas, NV
