Do I need a lift if I want my implant over the muscle? (Photo)

I had my 1st breat augmentation 21 years ago .400 cc mentor saline implant over the muscle .Was really happy with my result (round fake look).i hade 2 pregnancies after and breast got bigger and move to the side when i lay down. Do i need a breast lift if a i want my implant over the muscle again and do i need to go bigger implant for the round fake look

Your lateral pocket on the right side can be tightened so it sits a little more medial. I would suggest not going larger, or very minimally larger, for your tissue may not support the extra weight. If you go larger, you may need an internal bra of mesh.

Your implants are too large for your chest and the one with the nipple out to the side is pushed too far medically. Your breast bases are not symmetrical to start, so this adds to the problem. I would suggest going down in size and tightening your medial pocket on the side with the outward nipple.
