This requires revision of the surgery. To create a true plan, I would need to perform at least a virtual consultation. This surgery can be challenging including release of the fascial bands that form the double bubble, possible capsular tightening, and possible placement of an internal bra (Galaflex). I recommend finding a surgeon with 6 years of integrated plastic surgery residency and also a year fellowship accredited by the Aesthetic Society. Best of luck!
There are two causes of double bubble: Lowering the inframammary fold resulting in the implant dropping, and detachment of the pectoral muscle which is done with the dual plane method. The cut muscle can adhere to the capsule resulting in pulling along a crease that looks very similar to the first cause. If you see the crease pull up when you contract your pectoral muscle then it is the second cause, known as animation deformity. This requires muscle re-attachment. If not, then a capsulorrhaphy. Your implants are too large for your chest and the one with the nipple out to the side is pushed too far medically. Your breast bases are not symmetrical to start, so this adds to the problem. I would suggest going down in size and tightening your medial pocket on the side with the outward nipple.
With capsules, I don't recommend going bigger since your risk of capsulation increases. With thin capsules, they can be left in place and just be opened. Going up to 395 will increase you about one half cup in size.
Do not go bigger. Your tissue is not supporting the weight of your current implants and will not support heavier implants. You need to go smaller and may need an internal bra of mesh for better support. The inferior pocket can be tightened and raised back up so the scars are more in the crease.
Measurements are needed to see what is the widest implant your breast base can accommodate. However, I doubt you can go much bigger. I agree with both physicians' assessment.
