Hello, i planned my revision surgery next week. I have breast implants 6 years already 315 CC,and want to go for bigger (395-445CC).One surgeon said to me that he wants to remove capsule and put 445 CC.Another one thinks to leave capsule,because it looks thin and put 395 CC implants.I am so confused now, which one is the right decision about capsule? and how much breast will be increased in size if i will go from 315 CC to 395 CC?
Answer: Capsules With capsules, I don't recommend going bigger since your risk of capsulation increases. With thin capsules, they can be left in place and just be opened. Going up to 395 will increase you about one half cup in size.

Answer: Implant size Dear takoanato, breast implant type and sizing selection is a complicated process and requires a detailed consultation that includes a discussion of your goals, a detailed examination, and a lengthy discussion. But the decision will always start and end with the patient. When it comes to sizing, we have all of our patients find 3 photos of their desired breast size. We ask for naked breast photos because clothing can obscure the true size (push up bras). Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements.Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match.Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process. You don't want to invest time and money into a result that is either too big or too small.You should discuss your concerns with a board-certified plastic surgeon and make sure that your surgeon understands your goals. Answer: Being worried about revision surgery If you’re increasing the implant size, and the capsule needs to be cut in order to make room for the bigger implant. Individuals who develop capsular contraction benefit from having as much of the capsule removed as possible. If the capsule is thin and not causing a problem, then removing the capsule is not particularly important, but may serve some advantages. The removal of the capsule or not it’s probably not an important decision that you should be worried about. I suggest focusing your issues on the two variables that matter and those are your candidacy for the procedure in the first place and choosing the right provider. In the hands of the right provider, you’ll get the right treatment and if this includes removing the capsule or not removing capsule, that probably isn’t that important. Understanding and communicating clearly about what your wishes are in regards to final size is important, and the ability to do that well as one of the hallmarks of a good plastic surgeon.
