I am wondering if this is a realistic transformation and what should I look for in a rhinoplasty?

The change shown may be achievable, but an in-person assessment is needed to judge what your anatomy will allow. An edited photo is a useful way to communicate your goals, rather than a guaranteed result. The focus should be on refining the bridge and tip while preserving breathing and keeping your nose in proportion with your face.
Chin projection also affects profile balance. Depending on your examination, chin filler could be an optional complement to rhinoplasty. I treated a similar case using chin filler, and you can see that patient’s photos on my Instagram, as a reference&mdash;not a prediction of your result.Look for a qualified surgeon experienced in rhinoplasty, with healed results in patients who have similar anatomy.

Yes, you are an excellent candidate for a rhinoplasty and genioplasty. The length of your nose can be shortened, the dorsal hump resolved and the nasal columella angle corrected. A chin implant will add projection to balance your face.

The digital imaging you presented does appear very doable, but it's imperative to choose a surgeon with extensive experience with this combination. Rhinoplasty is the most difficult operation to perform correctly in the entire field of cosmetic surgery. A closed rhinoplasty approach can accomplish decreasing the overall projection of the nose by shaving down the dorsal hump, and the chin implant can add projection to your chin to actually make the nose appear smaller as well.

Thank you for your question. You are an excellent candidate for rhinoplasty and your expectations are realistic.  The chin implant would make an nice addition, however I would also place fat in the crease between the chin and the lip.  Sometimes, adding a chin implant makes that cleft much more prominent. It is one of the most beautifying surgeries that we do as plastic surgeons. A large portion of my practice is dedicated to rhinoplasty surgery. It is important to only see plastic surgeons with extensive experience in rhinoplasty, ie perform at least 6 per month and have performed hundreds of cases. Also, critically evaluate on-line galleries to see if their art form resonates with yours. Rhinoplasty is definitely a grey-haired plastic surgeon's sport. You should be able to have a beautiful result and would enhance your face dramatically. Best wishes to you.

Hello and thank you for your question. Based on your photographs, you are a great candidate for rhinoplasty. Maneuvers for your surgery would include dorsal hump reduction, tip refinement, nasal tip deprojection, tip elevation, among other maneuvers. A chin implant can safely be performed at the same time. Make sure you are going to a surgeon with hundreds of long-term before and after pictures on their website with standardized lighting and standardized backgrounds. With good surgical technique, you could have an outstanding result. I recommend that you seek consultation with a qualified board-certified rhinoplasty surgeon who can evaluate you in person.

Your photos give a general idea, but a goal image is only useful if your own anatomy can support it, so the realistic answer comes from an exam rather than from comparing pictures. Combining a rhinoplasty with a genioplasty is very common for a reason: the nose and chin are judged together in profile, and a weak or set-back chin can make a nose look larger than it really is. Often a modest change to the chin lets the nose be refined more conservatively. When choosing a rhinoplasty surgeon, look at many before-and-after photos of noses similar to yours, ask how they protect breathing, and make sure they evaluate the chin, lips and overall proportion rather than the nose in isolation. A consultation with a board-certified surgeon who does a lot of facial balancing work is the best way to learn what is achievable for you.
