Question

I am 23 male and there is a ditch between my forehead and nose ,i am having a bi maxillary case ,so how do iI fill that ditch ,is it because of maxillary recession and what are the treatments

Hello and thank you for your question. Based on your photograph, you may benefit from a radix graft consisting of diced cartilage wrapped in fascia. Make sure you are going to a surgeon with hundreds of long-term before and after pictures on their website with standardized lighting and standardized backgrounds. With good surgical technique, you could have an outstanding result. I recommend that you seek consultation with a qualified board-certified rhinoplasty surgeon who can evaluate you in person.

The indentation you describe appears to be at the nasal root, or “radix,” where the forehead meets the nose. Some concavity here is normal, and its appearance depends on the relationship between your brow, nasal bones, and bridge. It does not by itself indicate a recessed upper jaw, and that diagnosis cannot be made from this photograph.
If by “bimaxillary case” you mean a jaw alignment problem or planned double-jaw surgery, start with your orthodontist and oral and maxillofacial surgeon. Upper-jaw surgery can change the nasal base, tip, and overall facial balance, but standard jaw advancement does not directly fill the radix. Your jaw treatment plan should therefore be clarified before deciding on nasal correction.
If the indentation still bothers you, radix augmentation with a cartilage graft during rhinoplasty may soften the transition. Filler can also camouflage it temporarily, but this is a high-risk injection region, with rare complications including skin damage and blindness.
The goal should be a balanced forehead-to-nose transition rather than completely filling the natural dip. An examination and standardized photographs would help determine whether any treatment is appropriate.

The dip between the forehead and nose is called the radix, and when it sits deep it can make the nose look shorter and the profile less smooth. It is not usually caused by the upper jaw: bimaxillary protrusion means the jaws and teeth sit forward, which mainly affects the lips and lower face. A deep radix can be raised with a small cartilage or fascia graft during rhinoplasty, and filler is sometimes used as a temporary trial, though injections in this area carry their own risks. Because the brow, nose and jaws all shape the profile, an in-person evaluation with profile views, ideally coordinated between a rhinoplasty surgeon and an orthodontist or jaw surgeon, is the sensible next step.
