There are two variables that generally government whether someone gets a quality outcome from a cosmetic surgical procedure. These two variables govern the outcome of all cosmetic surgery. The first variable is the patient’s candidacy for the procedure. The second is the skill and experience of the surgeon. Most patients do not understand their own candidacy for breast augmentation. There are certain women who have ideal breast for implant augmentation, while others are going to have potential issues because of their own body. One such example is women who have divergent breasts. If the nipples pulling out towards the sides, instead of straight forward, like headlights on the car, then this will be amplified with the projection that implants create. This is only one exampleThere are lots of criteria that differentiates those who are excellent candidate from the store, average candidates, or poor candidates. Without pictures, we can’t begin to make an assessment. The choice of provider is more important than people realize. There are significant differences among board-certified plastic surgeons. Being board-certified, and plastic surgery with years of experience in an overall good reputation does not mean somebody has mastered any one single procedure or is even good add a certain procedure for that matter. To get a good assessment to understand your own candidacy for the procedure, including with the procedure can, and cannot accomplish and help find the best provider. My best suggestion is for patients that have multiple in person consultations with local providers. There’s no correct number of consultations needed to find the best provider. The more consultations you scheduled the more likely you are to find the best provider. In the hands of the right provider, the choice of implant shape, size and type will be explain carefully so that you have an understanding and knowledge in the decision process. The surgeon should guide you through this process based on having a good understanding of what you’re trying to achieve. Generally, speaking, smaller in plants create a more natural appearance since the ratio of tissue to implant leans more on breast tissue. if 95% of a woman’s breast is implant, and her breast are going to look like implants. that said, a significant number of women get used to the new breast size and sometimes wish they went a little bigger, despite the appearance, not looking passible as unadulterated breasts. Generally speaking, I prefer to work with low profile implants, but the decision is based on the breast, width and desired final outcome size Using the same diameter breast if a woman wants larger breast, then higher profile implants are indicated since they will have a greater volume with the same diameter. In the hands of the right provider, all of this becomes simple, straightforward and clear I cannot emphasize enough the importance of provider selection. Implants bottom out is a technical error when the procedure is done by those lacking sufficient skill and experience to get consistent quality long-term outcomes. While implants riding high it’s also a potential technical issue. It is far easier to correct than implants that bought them out. Patients who are uncertain or still have significant questions should probably either go back to their provider and get all the questions answered, or continue having more consultations until you feel very comfortable. You found the right provider for your needs. During each consultation, ask each provider to open up their portfolio and show you their entire collection of before, and after pictures of previous patients who have similar body characteristics to your own. An experienced provider should have no difficulty showing you the before and after pictures of at least 50 previous patients. A highly experienced surgeon shit in fact have hundreds or preferably thousands of before and after pictures to choose from. Being shown a handful of preselected images, representing the best results of a providers career is insufficient to get a clear understanding of what average results look like in the hands of each provider or how many of these procedures they’ve actually done. I recommend taking careful notes during each consultation, especially regarding the quantity and quality of before and after pictures. Always ask surgeons what their revision rate is for each of the major complications that can happen after this procedure, especially capsular contractions, adjusting the position of the implant and changing implant size or shape. Make sure you have a clear understanding of each surgeons revision policy. Breast implant augmentation has a fairly high re-operation rate so these discussions should be had. Do not make assumptions that surgeons know what they’re doing, that things will be fine or that you won’t have a complication or need a revision. It is your responsibility to properly vet, providers, and interview enough surgeons to have a sense of comparison. Best, Mats Hagstrom, MD
