Should I consider a lift in addition to round implants?

Question

After having 2 pregnancies, breastfeeding and recently a 25 pound weight loss I have deflated breasts. I would like to schedule ba surgery with a tummy tuck, lipo and labiaplasty. I have 3d simulation with Motiva ergonomix. I told him I would like to have natural breasts. I asked for a 3d simulation with round implants so I could compare. His assistant responded that I will have to ask him about that during our next meeting. Would my breasts need a lift with round implants?

Your before photos show that you probably have a Grade II breast ptosis and in my opinion would benefit from a breast lift.  On idea would be to do the augmentation first and see if you were happy with that result.  You could do the lift at a later date if you felt you needed it.

determining whether you need simply a breast augmentation versus a breast lift can be somewhat complicated. It depends on a number of factors including skin laxity and current nipple position. Generally speaking, if the nipples are lower than the inframammary line the patient will most likely need a breast lift. If the nipples are at or above the inframammary line we can generally get by with just a breast augmentation depending on the size of the implant the patient wants. I often still using implant when I perform a breast lift because it provides more upper breast fullness after the lift. To be sure a consultation with a board-certified plastic surgeon will help determine what the patient actually needs.

Thank you for submitting your question and photos. It appears you have grade 2 ptosis (breast sag) secondary to your pregnancies, breastfeeding, and recent weight loss. Normally when breasts have fallen to grade 2, a lift is needed to reposition the nipple areola complex in a more central position on the breast mound to achieve a youthful look. Implants alone will not be enough to reposition the nipple in a higher position. Be sure to consult with a board-certified plastic surgeon.

The technique I recommend is a mini lift. Using a circumareola incision, your breast tissue is reshaped, elevated higher on the chest wall and more medial to increase your cleavage. At the same time, fat transfers or small implants can be placed.

After I measure and discuss goals with my patients, we then look at pictures of women (my patients) with similar measurements with different size and style breast implants. They also get to see what they would look like when the breasts have fully dropped and fluffed and in clothes. You get to see actual results and the surgeon's experience and not a virtual idea of what can be achieved. Women tell me that this process is very helpful in determining what size and style breast implant is right for them. Mildly Sagging Breasts: If your breasts are mildly droopy, a saline or silicone gel implant placed behind the pectoral muscle might help create the illusion of perkiness. That’s because as the implant fills out the top of the breast, it also fills out the bottom, making it look as if the nipple has moved higher. It hasn’t. Just based on photos only and not measurements, if the implants are placed behind the muscle, you may need a lift. If the implants are placed in front, you might be happy with no lift and a more "natural" look. However, long term, you will have more sag from the weight of the implants on your sagging breast tissue.  I recommend going behind the muscle and be measured to see if a lift is needed.

Based on your photos it appears you would have the best outcome from a breast lift with implants.  This would improve asymmetry, raise the position of the nipple, and give you the extra volume from the implants.  I've included a video that I hope you find helpful.
