I have hollowness in central neck after facelift/neck lift. What can be done? (photos)

This is due to aggressive liposuction. Talk to your plastic surgeon about a revision of the platysmaplasty. He or she may need to reapproximate the platysmal bands, or excise the bands further, as well as redrape and tighten the neck skin laterally. Revision surgery is ok, it happens in plastic surgery. Unfortunately recurrent banding and a hallowed appearance submentally can occur after a facelift, especially with aggressive liposuction. That said, this can be corrected by a revision procedure. Usually done after one year, this would involve suturing the platysma muscle (which creates the bands) back together. In addition, fat grafting or placement of fillers into the hollowed areas would be beneficial as well.Unfortunate that you are having this, but definitely something that can be corrected-

From the photograph it looks as though the platysma muscle is still by aesthetic after the facelift. A physical examination would be necessary to substantiate this. If this is the case the platysma muscles will need to be re-approximated surgically.

It appears that the platysmaplasty split open where the two cords are sewn together. This can be corrected suegically by having the platysmaplasty redone.

The area of concavity in your submental area is a form of cobra head deformity from liposuction. It can be resolved by using the same submental incision and repairing the muscle layer directly in the area of the deformity. Fat grafting in these areas after extensive liposuction does not work well but the muscle repair is very helpful. I would leave your salivary glands alone as your primary issue is the central concavity.

In this case it is important the physical examination to determine which structures in your chin are involved in this depression, but on the other hand, could experience the following options for handling management:1) make a new platysmaplasty by the prior chin scar. 2) Using fat grafting for correct and fill the chin depression.3) platysmaplasty +  fat graft (recommended.Regards,

Share your concerns with your surgeon, so all of your options can be discussed. Thiese appears to be your submandibular glands. They can be suspended to reduce their appearance. If there is some platysmal laxity, this too can be remedied.

 the issues in your picture may be due to overaggressive liposuction under the chin. This can be corrected with fat grafting after a period of healing. The areas to the sides which show some fullness appeared to be ptotic  submandibular glands. These can be resuspended in certain cases. A full examination is required to truly determine the nature of your problems, see a board-certified plastic surgeon for help.

Your hollowness in your photos appears to be from the submental liposuction.  This should be able to be corrected with fat grafting to the area. It is important to express your concerns with your plastic surgeon.  Hope this helps!
