I would recommend a revision rhinoplasty with rib graft or rib allograft (from a deceased donor). You need a very complex surgery to address the issues going on with your nose. This can only be tackled by a very experienced and surgery willing to tackle these problems. It is absolutely critical to find an American Board of Plastic Surgery board certified plastic surgeon with experience in rhinoplasty, one preferably with an accredited Aesthetic fellowship endorsed by the Aesthetic Society.
Hello and thank you for your question. Based on your photographs, that is not an inverted V deformity. It is an indent due to lack of structural support in the soft triangle of your nose. This can be fixed with a revision using extended alar contour grafts, among other maneuvers. With good surgical technique, you could have an outstanding result. I recommend that you seek consultation with a qualified board-certified rhinoplasty surgeon who can evaluate you in person.Best wishes and good luck.Richard G.
