The thickness of the skin covering the labia majora is about 4 millimeters and the incision is done just beneath the dermis. Above the subcutaneous fat of the labia majora.
The depth of the incision is just below the dermis. This varies between 3-5 mm. The subcutaneous fat is left in place. There are 3 methods for labia majoraplasty. First is RF shrinkage, which requires 3 treatments, no scalpel, shrinks ~ 50% costs ~ $3000 and must be repeated (series of 3) every 8-10 months. The second is a "double hockey stick" procedure, performed by Dr. Dawid Serafin in Poland, and myself here in the U.S. which requires 2 "double chevron" shaped incisions in the mid-lower labia majora, and does not so much remove a lot of skin, as it "lifts" saggy, older-appearing labia majora. The third, what you ask about and most utilized, is the vertical "half-moon" or "teardrop-shaped majoraplasty which removes the epidermis (superficial skin) and deeper dermis (lower, generating layer) and superficial fascial layer, and- as others have said- removes ~ 2-4 mm thickness. The important thing is to not disturb the underlying fat pad and its thin fascial covering, along with a careful monofilament subcutaneous or removal nylon mattress closure, so as to produce only a very thin scar.
