Labiaplasty and keloids?

Question

I'm looking to get a labiaplasty. However, I'm worried about keloid scaring. I'm very prone to it. I've had other surgeries/injuries where all the incisions became a keloid. I'm currently having steroid injections. It works on some scars, not all. Should I still get the labiaplasty or just skip it? I'm worried about getting a keloid and causing myself further pain. I would hate to be stuck getting steroid injections for a long time trying to flatten the scar, especially in that area. Thank you.

In my experience, labiaplasty on the labia minora has not been associated with keloid formation. However, keloids may develop on the labia majora. It is important to consult a cosmetic gynecologist that has experience in labiaplasty of both labia minora and majora.

Thank you for your questionsAll surgery has risks and the decision to do or not is all yours. There are several techniques to labiaplasty and risks may vary depending on which is used. I have not seen a keloid with a labiaplasty in almost twenty years of performing them but I principally use posterior delamination , contour trim and fenestration techniques. Do your homework and see if your surgeon has ever had a keloid with his labial surgery. Look at the extent of their training in labiaplasty techniques, examine his/her reviews as well as lots of before and after photos. The video below may help.

Keloids don't form on the labia minora. They will, however, form on the labia majora. Most techniques of labia minora reduction labiaplasty don't extend to the labia majora. If you want to be as safe as possible, make sure to see an expert in these procedures and insist that they don't extend the work to the labia majora.

While keloid formation is possible anywhere on the body, the labia minora are actually one of the least likely areas to develop keloid scars. This is because the tissue in the genital region is very soft, flexible, and has excellent blood supply, which typically supports smoother healing and less aggressive scar formation compared to areas like the chest, shoulders, or back.

That said, since you have a personal history of keloids, we will take extra precautions. We’ll carefully assess your risk during a consultation, and if you decide to proceed, we can:

Use a meticulous, layered closure technique with the least tension possible

Recommend post-operative scar management, such as silicone gel or steroid-based treatments

Closely monitor your healing and intervene early if any abnormal scar activity begins

While the risk is low in this area, your history is important to consider—and we’ll partner closely with you every step of the way to ensure the safest and most aesthetic outcome.
