I do not recommend performing a sliding genioplasty and chin implant simultaneously. The greatest risk of problems with a chin implant is infection and the oral cavity is not a sterile environment. I usually perform the genioplasty and 6 months later place the implant through a submental incision. However, I think that you can accomplish vertical lengthening and making the chin more square with just the sliding genioplasty alone. In these cases, I perform a "T" osteotomy and make the anterior chin wider. It looks the most natural and doesn't require a second surgery. The surgery is performed under sedation anesthesia in the office.
