Lower extremity liposuction is technically difficult, and often does not deliver as much of an improvement as patience at home before. If done by providers, lacking expertise in the procedure, patients can be left with disfiguring contour irregularities. Breast fat transfer is a subtle procedure that will give most patients 1/2 cup breast volume increase. It does not change the shape of the breast in any significant way, simply adds a very subtle amount of volume. I cannot emphasize enough the importance of provider selection when it comes to this procedure both liposuction and fat transfer. The number of plastic surgeons will have the skills to deliver, consistent quality outcomes, including the ability to make an accurate assessment and deliver predictable outcomes is a relatively small. Being board-certified and plastic surgery with an overall good reputation and years of experience does not mean that somebody has mastered Liposuction, or is even competent at Liposuction. The same is true for fat transfer. Doing these procedures, well, on a consistent basis with the ability to accurately predict outcomes, is much more difficult than most people believe. Most plastic surgeons think they’re better at Liposuction then they actually are and the number of providers who are excellent is really a select few. Because the implications of having poorly done liposuction is devastating any reversible patients need to think long and hard if this is something that’s justified. If you decide that you want to go through with the procedure, then you must carefully select to find the best provider and get a clear understanding of your own candidacy for the procedure. To do this I recommend patients have multiple in person consultations. Bring pictures of your own body, including of your breast to the consultation in printed format to use for reference. During each consultation, ask each provider to open up their portfolio and show you their entire collection a before and after pictures of the areas you’re interested in treating. It’s much more common to do Liposuction of the Torso or abdomen for breast fat transfer, but fat can be harvested from any part of your body. Ask providers to show you as many pictures as possible of previous patient who has very similar body characteristics to your own. This is especially true for Liposuction. For reference and experience plastic surgeon should have no difficulty showing you at least 50 sets of before, and after pictures of commonly performed procedures, like liposuction and breast fat transfer. When reviewing before, and after pictures, make sure patients have very similar body characteristics to your own. Have pictures of your own body available to use as a reference. Preferably imprinted format. I look very carefully at all areas treated with Liposuction. Make absolutely sure there are no contour irregularities in any area treated with Liposuction. Asked providers to point out exactly what areas were treated with Liposuction and what areas were not treated than look very carefully at transition zones between treated and untreated areas. For fat transfer results do you need to make absolutely sure you know what the timeframe is of when the after pictures were taken in relationship to the time of surgery. Early fat transfer results can look extremely impressive, but do not in anyway represent final results which take at least 3 to 6 months and are generally far smaller and less impressive than early results. There’s no correct number of consultations to have to find the best provider. The more consultations you have the higher the chance of finding the best provider. Considering how few plastic surgeons are truly exceptional at both Liposuction and fat transfer this is going to take some serious effort if you want to minimize the chance of having a poor outcome, including becoming disfigured. Honestly, I think your body looks great and you should stay far away from plastic surgery offices. Best, Mats Hagstrom, MD
