Hello and thank you for your question. Thebest advice you can receive is from an in-person consultation. With thatbeing said, based on your photographs, you do have tuberous breasts with asymmetry. The best shape and contour could beachieved with an implant and a breast lift on the larger side and an implant with release of lower pole constriction on the small side. The size, profile, and shape ofthe implant is based on your desired breast size/shape, your chest wallmeasurements, and soft tissue quality. This decision shouldbe based on a detailed discussion with equal input from both youand your surgeon. Make sure you specifically lookat before and after pictures of real patients who have had this surgeryperformed by your surgeon and evaluate their results. The mostimportant aspect is to find a surgeon you are comfortable with. Thank you for your question. Selecting the correct implant is among the most important decisions in a successful breast augmentation procedure. There are several factors to consider and the placement of the identical implant will not produce the same results in all patients due to differences in:1. Dimensions of the breast 2. Diameter of the chest 3. Volume of the breast tissue 4. Height and weight 5. Breast shape, including ptosis and asymmetryI rely on 3 methods to advise my patients: 1. Dimensional planning 2. Gel implant sizing 3. 3-Dimensional Vectra imaging system.At the time of consultation, measurements are taken allowing for dimensional planning to select the implant which best fits the base diameter of the breast. Gel implant sizing is performed to allow you to visualize augmentation with the various sized implants. Vectra 3-D imaging has been a very important method in my practice in selecting the correct implant size and shape as it allows you to see what you will look like following augmentation with any of the available implants. Your problem is relatively uncommon, but fortunately not for an experienced plastic surgeon. Breast asymmetry in mild forms is extremely common, but obviously yours is more significant, but this is correctible, and you should be able to end up with a very pretty outcome. There are many ways to find an experienced plastic surgeon but it does require a bit of research. Making sure your plastic surgeon is either board eligible or board certified is critical. Both the American Society of Plastic Surgeons and the American Society for Aesthetic Plastic Surgery are a good place to start. Google is good as well, but listings on Google are often "paid advertising". Most important are the credentials and training of your plastic surgeon, but also his or her sense of aesthetics needs to align with your own. Be open to the idea of more than one consultation or opinion before singing up for surgery. As far as a specific surgical treatment plan, based on your photos alone, you will need at the very least a breast lift and/or reduction of your right breast to make it resemble your left breast if you do not want your left breast augmented. If you do want your left breast augmented, you may or may not need to have the right breast augmented as well, depending on the final breast size you hope to be. Since you are only 5 feet tall, you do not want your breasts to be disproportionately large for your frame. Use photographs of your concept of "ideal" to communicate with your plastic surgeon as to the final outcome you are hoping for. Best of luck to you.
There probably is not a plastic surgeon who "specializes" in tuberous breast deformity since it is not that prevalent and is relatively easy to reconstruct. There are a few characteristics common to that condition including a constricted lower pole, herniated breast tissue, dilated areola, tubular or narrow breast base, elevated inframammary fold. These conditions can occur alone or in combination. Contact a real plastic surgeon who is board certified by the American Board of Plastic Surgery for a consultation. Since there is no way to say definitively over the internet which size would be optimal for you, take comfort in the fact that the surgeon uses your unique measurements and is able to provide you with a general range. In many cases the board certified plastic surgeon can best be your guide as they’ve met with you in person and heard your aesthetic goals. You may consider doing 3-D imaging; however with nearly all the breast cases we’ve performed, we’ve found patients enjoy doing a live “try on” session where they actually can “try on” the implant in a non-padded bra in our office. This does not give an exact measurement of outcome, rather it gives a nice idea of what to expect. For patients who desire a more natural look we like to recommend the shaped implant. For patients who desire a more full, voluptuous look we recommend a higher profile implant (like Inspira). Remember that all breasts have at least some amount of asymmetry (we like to say they’re like sisters and not twins). An in-person consult is the best way to discuss your questions.
