Hello, as I've aged, the sagging on one side of my face has become more pronounced, creating a deep line around my nostril as I smile. I would like to address this and soften it in the near future. I read about threads to "lift" the cheek but I'm hesitant due to the negative reviews. Can you please advise your recommendation on how to go about achieving symmetry and lifting in my mid-face
Answer: Advise My number one piece of advise would be to absolutely never do thread lifting. It is a blind-maneuver with minimal effect that certainly does not last. In addition, this makes future facial surgery incredibly challenging due to the internal scarring and foreign body reactions seen with the threads. I would recommend seeing a board-certified plastic surgeon (by the American Board of Plastic Surgery) to further discuss your deepened nasolabial folds. This is usually the result of descent of the adjacent cheek causing the elusion that the fold has deepened.

Thread lifts do not work very well and have many potential complications. I would start with fillers, such as juvederm of fat transfers. They can significantly improve your asymmetry. These procedures have little risk, and last 1-2 years. A mid-face lift is a surgical option, but you appear too young for this procedure. I would recommend to start with fat transfers.

A mid-face/cheek lift can help with softening of the naso-labial folds that you are looking for, with a small cut and limited dissection. I would recommend getting it done on both sides with a little more lift on the side which is more saggy. The improvement is significant and long lasting unlike thread lifts.
