I’m a pretty small person, 41 years old, 5’4” 115lbs. Beast fed 3 boys. I’d like to improve the fullness of my breast and perkiness (and not have to wear a bra if I don’t want to), but not big boobs. I’ve been told I do need a lift and I don’t. I’m worried I pick the wrong route and wish I hadn’t don’t any of it. I also want my abs, flanks and Bra line liposuctioned as I am self conscious of them. 360 Lipo/fat transfer?

Answer: You're not going to like what I have to say... 1. Breasts. You have enough ptosis and breast asymmetry to warrant a lift. Nothing shy of this will give you what you want; neither auto-augmentation (during a lift) or fat transfer will sufficiently fill the upper pole of your breast or give you a perky breast. A relatively low profile, and appropriately wide silicone gel implant will do the job of upper pole fullness without excess volumization of the entire breast. The larger breast will be reduced accordingly to match the smaller breast volume. 2. Body: Although you may have enough fat to benefit from liposuction, you need a tummy tuck. You have too much skin laxity, and liposuction will make that worse. Additionally, the frontal photo shows a mass just to the side of midline between ribcage and belly button. I assume its a port to a lap band? This can be removed if desired with a tummy tuck.

Answer: Bellesoma Method Your breasts are low on the chest wall. I recommend a breast lift using The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. Later, fat transfers can be performed if additional volume is desired. Best Wishes, Gary Horndeski, M.D.
