Best advice is to select a Board Certified Plastic Surgeon, and check on plasticsurgery.org to confirm by entering the name. Secondly, ask how many the surgeon has done, and how many in the past year. You want the answer to be hundreds at least in total, and at least 20 in the last year to insure experience and that the surgeon still does it. Lastly, look at before and after pictures, make sure they are similar in size and ethnicity. They should show you at least 3. Good luck
It is not uncommon to have some numbness on the lower half of the breasts, and unfortunately, loss of nipple sensation is a risk of the procedure. The sensation of your nipples being where they used to be is puzzling. Perhaps did you have very large breasts and the doctor did Free Nipple Grafts? I would ask that question and return to your surgeon.
Axillary breast tissue can be removed by liposuction or direct excision, depending on the amount of extra skin. In your case, direct excision with the line being placed in the armpit would be my advice.
The limit to size in Breast Reduction depends on how low your nipple sits. If the goal is to preserve sensation and circulation, a pillar of tissue needs to be preserved from the chest to the nipple. The lower the nipple, the longer the pillar, the less you can reduce. The other option is called Free Nipple Grafting, which enables you to go as small as you wish, but you lose sensation and ability to breast feed. Photos would help.
If you are uncomfortable in your back and neck, there is no reason to wait longer for a secondary Breast Reduction. After that much time, almost any technique may be safely used. Do speak with your GYN about HRT, and make a plan surrounding the procedure.
