I had an open septorhinoplasty 20 months ago. My nose is constantly leaking a clear fluid (SCF check came negative), the tip is still bulbous and I got a very visible deep hole on the side of my nose. Also some dent is going across of my bridge, right In the middle, making an appearance that the tip of my nose starting from the middle of the nose. And I got a bulge on side of the tip and I can see the septum separation on my bridge. Is it even possible to fix now ? (Please note I’m 45 yo).

Answer: The feasibility of revision rhinoplasty would require an examination.
I would just recommend a rhinoplasty. Most of the "bump" is actually cartilage. The nasal bones merely have a "bony cap" extension over the most prominent part of the cartilage. Usually rasping only removes the bony cap and other measures are needed to fully address the dorsal convexity.
Let's speak about this. The rib graft pocket is above the bone it seems. This makes the graft mobile. We can likely address this in the office with local anesthesia.
I think you identified what changes would benefit your nose. The tip is more projecting than it needs to be. Then, when a nose is de-projected, it tends to make the nose *look* longer, as if the tip had drooped, so it's *very* important to elevate the tip at the same time.
The good news is...
