I want to downsize to 200cc to a more natural look. Do I need a lift? I'm Asian. Will my scar be really bad afterwards?

Dear Bella,   Thanks for submitting your pictures, however full front view and right lateral view would have been helpful.   Sorry for your problems and unsatisfactory outcome of your 2 surgeries .  From observing your pictures. you appear to be C or small C cup size. Reducing your implants size to 200 cc, will bring your to a B cup size, which hopefully will make you happy.    In addition to the breasts size issue, you have also other problems  with, at least, the right breast -  lateral displacement of the  implant and capsule contracture medially, creating a wide cleavage. You do not have ptosis (droopiness), so a lift procedure is not necessary.    When you replace the implants, I would recommend, for better outcome, to have your surgeon perform lateral capsulorrhaphy (internal bra) and medial capsulotomy (scar release) on the right breast, for the creation of nicer cleavage and more natural look.   Always, consult with experienced board certified plastic surgeons who operate in accredited surgery center for your safety. Most importantly, check the before and after pictures of redo breasts augmentation in the photo gallery, to make sure that they are numerous, consistent and attractive .                        Best of luck,                                               Dr Widder

Without pre-op photos, benefit of physical exam and reading your op-reports it's difficult to comment accurately. Thorough communication with a Surgeon is always recommended prior to undergoing a surgical procedure. I suggest that you return to your Operating Surgeon and discuss your concerns. 

Really there is no way to tell for sure if you will be happy with the position of your nipple and breast after you downsize the implant.  The right size for your implant and whether or not you wanted to the lift needs to be determined with a long conversation with your surgeon.  You can always try downsizing and lift at a later time if needed.Best of luck!

Yes you will be charged for at least new implants, surgery room costs and anesthesia fees, + if adding lift than the MD would add his fee. Best to seek more in person opinions.. 

Honestly, going down 25cc is a minimal change--it's always important that you finalize implant size with your surgeon prior to surgery because once they are in they are yours and can't be returned or exchanged unless there is a manufacturer defect or you buy new ones.  That being said, they look nice and don't look too large based on the photos.  If you feel they are too large you may just want to have them removed all together and have a lift; possibly consider doing a little fat grafting for a modest augmentation.  Lots of options just depends on your budget and discussion with your surgeon.  Good luck!

You can consider the implant exchange first and then see how the breasts respond. A lift can always be performed at a later time.

Typically, to notice a difference, a change of about 100 cc is helpful.  It does not appear that you will necessarily need a lift, but that is difficult to determine without examining you.  

Hi and thank you for the question. I'm sorry you have not had a great experience with your breast surgery. Changing implant size by 25 cc's isn't enough to really change your breast size in any significant way. When my patients want to notice a significant increase or decrease in breast size, I typically suggest they change by 150-250 cc's. Of course it depends on your breast size as well as your implant size. If you drop down to a 150-200cc implant you will probably end up with some skin laxity that will be best addressed with a breast lift. Based on your current nipple position it is possible that the lift could be limited to a scar that goes around your areolas. These scars usually hide very nicely because they are at a transition point from dark areola skin to lighter breast skin. Consider getting a second and third opinion before you do your next surgery. I think that it is most likely, that if you downsize to 200 cc, that breast lifting will not be necessary.Generally speaking, the policy regarding coverage of expenses for revisionary surgery will vary from office to office and probably on a case-by-case basis as well. Costs of revisionary surgery may range from zero, to anesthesia/surgery center fees, to discounted surgeon's fees.
 Also, I generally speaking, I suggest patients communicate their concerns in a calm and constructive fashion; this form communication tends to be much more effective than confrontational communication.

I am sorry that you just went through a revision procedure.  I don't think there is a big difference between a 300cc. and a 275cc. implant in terms of the size differences.  I do think, however, that there may be a nice difference if you went down in size and used a teardrop shaped implant that may give you the shape and size that you might like.  Your surgeon may be able to discuss this with you in the future.  If you have any concerns that remain unanswered, you can always have second opinions expressedGood luck to you.Frank Rieger M.D.  Tampa Plastic Surgeon
